Bee Pierced on Commercial has pretty decent people, although I think they changed their name. Dark Star is decent. Addictions. The major, well-tended places are your best bet.

I don't have any recommendations for Salem itself, but Matt at Sacred Art Tattoo in Corvallis did a great job on my tattoo (see icon).
