(a) 
Permit Information.
A plot plan shall be prepared to scale and submitted to the building official showing the following information:
(1) 
All proposed driveways and sidewalks shall be shown in detail.
(2) 
Existing conditions between the right-of-way and the traveled roadway, including all curbs, ditches, storm sewer inlets, manholes, utility poles, fire hydrants, trees, etc.
(3) 
When private property being developed fronts a “T” intersecting street, the complete intersection shall be shown on the plot plan.
(b) 
Sidewalks Required.
Sidewalks shall be required to be constructed along an existing curb-type major thoroughfare abutting the property in all residential and commercial areas being developed.
(c) 
Permit Fees.
Fees to construct, reconstruct, repair, or regrade any sidewalk in the city:
(1) 
For the first lineal 100 feet, $5.00; for each additional 100 feet or part thereof, $3.00.
(2) 
Double Fees.
Where work for which a permit is required by this article is started or proceeded with prior to obtaining said permit, the fees above specified shall be doubled, but the paying of such doubled fee shall not relieve any person from fully complying with the requirements of this article in the execution of the work nor from any other penalties prescribed herein.
(d) 
Contents of the Plot.
The plot to accompany this request for extra length culvert pipes shall include:
(1) 
Location of proposed culvert pipe by street address.
(2) 
Size and length of culvert pipe approved or requested.
(3) 
Construction and spacing of type “D” inlets drawing no. 401.
(4) 
Conformity with driveway geometric standards drawing no. 402, regarding spacing of driveways for the construction of the concrete curbed island.
(e) 
Permit Fees.
(1) 
Driveways (first driveway), $8.00.
(2) 
For each additional driveway, $4.50.
(f) 
Driveways Prohibited.
Driveways are prohibited within the following areas:
(1) 
Within the areas set forth by the state transportation department as “access denied.”
(2) 
Within the areas designated “access denied” on record subdivision plats or any other plat required to be approved by the city planning commission.
(3) 
At the end of any dead-end street not terminating in a cul-de-sac or permanent turn-around and intended to be extended in the future.
(4) 
Within the limits of any intersection, with the exception that special consideration will be given to major thoroughfares with existing esplanades and streets primarily used for residential use.
(5) 
Within any area with less than twenty feet of unobstructed depth from the right-of-way line to any obstruction.
(6) 
Within any area abutting a major thoroughfare where the general design of the parking does not provide the necessary depth to allow a vehicle when exiting to enter the thoroughfare in a head-out position.
(7) 
Any existing driveway that may fall within an area where the twenty foot minimum unconstructed depth is shortened by the construction of a structure of any type. Such driveway shall be removed and the area restored to its original state.
(8) 
Within any area where the city inspector finds that it would not provide reasonable access to the private property consistent with the safety and convenience of the traveling public, in accordance with the terms of this code.
