Replaces os.Getenv("HOME") with os.UserHomeDir() in both main.go (doctor command) and watcher.go (project lookup).

HOME is not set on Windows by default, so the ~/.claude/projects/ path was resolving incorrectly. os.UserHomeDir() handles this cross-platform (uses USERPROFILE on Windows, HOME on Unix).

Together with #5 and #12 (path encoding fixes), this completes Windows support for project directory resolution.

Closes #4, closes #10

os.Getenv("HOME") returns empty on Windows, breaking the
~/.claude/projects/ lookup. os.UserHomeDir() works on all platforms.
